Please print the 2013-2014 FCHS Bylaws, Constitution, and Application & Teacher Recommendation Forms (HERE).
FOLLOW THESE DIRECTIONS:
11.) Please print all 22 pages
2.) Parent and Cheerleader, read and initial all pages indicating you understand and agree with the information on that page.
3.) Sign appropriate pages and put the entire packet in a decorated folder with prongs.
4.) In the folder, include a current physical, a copy
of the last report card, a copy of your insurance card, and all 4 teacher recommendations in sealed envelopes.
Please have your folder prepared to hand in on Monday, May 20th!
Please wear workout attire (t-shirt, shorts, sneakers, etc.) You will not be able to participate in the wrong attire.
